Application Form
An application
form for applying for a Guyana passport in the United States of America may
be obtained at a cost of US$1 from the Guyana Embassy or any Guyana consulate.
To apply for a
passport, please complete the passport application form carefully and sign in
the places indicated at the bottom of page 1 as well as section 8 on page 3.
Please note that a passport is valid for 15 years from the date of issue.
The main points
to be noted are:
- Three (3) full face black and white or color passport size pictures taken
on a clear background are needed. No machine or Polaroid prints are accepted.
- The cost of a new Republic of Guyana Passport (containing 32 pages) is
US$60 and US$120 for a "business" passport (containing 64 pages).
Payment should be sent in the form of a Money Order or Bank Draft payable
to the Embassy of the Republic of Guyana. An additional US$1 must also be
paid as cost for the passport application form. (There is a fee of US$35
for express service. This is optional).
- It will be necessary to send a small sum to cover registered return postage
of documents. If you live in the United States of America, this amount would
be US$6. This sum may be added to the fee for the passport and or Money Order
or Bank Draft sent. PERSONAL CHECKS ARE NOT ACCEPTED.
- You must submit your old passport, along with the application form and
Money Order/Bank Draft.
- Application for a replacement for a LOST PASSPORT must be accompanied by
a sworn notarized statement explaining how the passport was lost. According
to the Passport regulations, a period of six (6) months must elapse before
a lost passport is replaced. (However, a replacement is generally issued before
that time span). The cost of a replacement passport (32 pages) is US$100 and
for a "business" passport (64 pages) is US$200. (There is no express
service for the replacement of lost passports).
- Application from persons under 18 years of age will not be entertained
unless they have written consent of the father or other legal guardian. This
requirement also applies where the child is to be included on the passport
of his/her mother, brother, sister etc. There is a fee of US$10 for the inclusion
of each child's name and the birth certificates must be submitted.
- In the case of a woman whose name has been changed by marriage, the production
of the marriage certificate and her husband's birth certificate or his passport
are necessary.
